Which forms do I need?

You’ll need to complete the registration form before your session.

Where can I download forms?

All forms are available here for easy download and printing.

Do I need to bring signed forms?

Yes, please email your completed and signed forms to your Guide at least 24 hours prior to the session. It is not necessary to bring another form to the session if you have sent it to the Guide beforehand.

Can I fill forms online?

Currently, we require printed and signed copies to be emailed.

What if I forget forms?

We can provide blank copies on site, but it may delay your session start.

Can I discuss any of my information on the form before the session?

Yes, you can always contact your Guide prior to the session to discuss any concerns or questions that you may have.
